If I were you, I would try contacting the State of Florida Department
Of Insurance and asking them if they maintain a database (or other
records) of which health-care benefit provider businesses use.
Even if they do maintain such records, this doesn't mean that they will
necessarily tell you -- let alone provide you with (or give you access
to) these records.
Still, my guess is that it will be easier to get the records from the
business (client) end of things rather than from United Health-care.
In any case, no matter which agency (or company) has the information you
want -- you may have to resort to filing a request under the Federal
Freedom of Information Act in order to get access to it.
